New Patient Registration

Patients can register by calling in at the surgery. You will need to bring photographic proof of identification and attend in person if aged over 16. Children can be registered with a birth certificate if they do not have a passport.

All adult patients are required to complete a health check using the Surgery pod in the reception area. On occasion, your medical condition may require you to have an appointment with a healthcare assistant.
